Community Connector

Overview
SWN are supporting Derwent Valley Community House to recruit for a 'Community Connector' to participate in the 'Community Connector Program (CCP)'. The CCP aims to enhance the capacity of the House to address complex individual, family, and community issues by connecting people to appropriate programs, services and resources.
Requirements
Degree in Community and/or Social Work (or equivalent training/experience).
Professional experience working in Community, Social and/or Mental Health Work.
Working with Vulnerable People (WWVP) Check.
Willingness to undergo a National Police Check.
Willingness to undertake relevant training as required to support your role.
Current TAS drivers licence (essential).
Ability to engage with individuals, GPs, service providers and community groups.
Knowledge of and ability to raise awareness of mental health and wellbeing services.
Attending local events and community forums to promote the program and build networks.
Advocating for more integrated and connected service delivery across sector.
